Enroll your child
Enrollment is done on paper. Download the enrollment packet, fill it in, and return it to your site registrar.
How to enroll
- Download and print the packet. Complete one packet for each child.
- Fill it in and sign where marked. The immunization certificate must be completed by your child’s doctor or clinic, or you can attach a copy of the official immunization record signed or stamped by the provider.
- Return the completed packet to the registrar at your site. You will then receive an invitation to the ProCare parent app.
Every child needs a signed contract and current registration documents on file in the office before their first day.
